Protected Health Information

FindRx, Inc may request that you and/or your clinician provide Protected Health Information to enhance FindRx, Inc's ability to serve your needs and facilitate your use of its Services. FindRx, Inc may also use or disclose this Protected Health Information with your clinician. However, providing Protected Health Information through FindRx, Inc's Services is entirely voluntary. Failure to provide requested Protected Health Information may limit your ability to access certain features of FindRx, Inc's Services.

 

Accessing and Correcting Your Personal and/or Protected Health Information

FindRx, Incs’ Services exist to enable you and/or your clinician to measure treatment adherence, response, progress, and outcomes.

  • To present and facilitate FindRx, Inc's Services to you and/or on your behalf.

  • To provide you with information, products, or services that you request from FindRx, Inc.
